### Question

Hi,

For approximately the past 15 days, we have been experiencing issues with forms in the Epicollect5 application, specifically with the date and geographic location fields, which do not allow us to enter or save information correctly.

It is important to note that this issue has occurred across multiple devices. We have attempted to resolve it by updating the application and restarting the devices; however, the problem persists.

This situation is making it difficult to complete the forms and accurately record the information collected in the field.

We would appreciate your assistance in investigating this issue, identifying its cause, and advising us on the necessary steps to resolve it, as it is affecting our data collection activities.

We look forward to your feedback and recommendations.
